I believe that the Israel and Palestinian Gaza conflict is a false flag operation being carried out by Illuminati family’s and Satanic secret society’s to set up World War 3.   The Illuminati family’s control and have infiltrated just about every facet of society they do control key Government offices as well as private corporations through a type of iron triangle. In United States politics, the iron triangle comprises the policy-making relationship among the congressional committees, the bureaucracy, and interest groups.[1]  Illuminati Satanic secret societies, have been behind a wide range of events and disasters going as far back in history as the French Revolution and possibly even farther.

It is a historical fact the Hamas was founded by the Israel government Israel may want to destroy Hamas now. But it played a key role in the group’s creation. According to JerusalemOnline.com  “Israel actively supported Hamas” Documents show that Israel was interested in empowering Hamas in attempt to end the first Intifada. During Operation “Protective Edge”, news leaks website WikiLeaks exposes secret documents which were passed between American diplomats in the 1980’s. These documents allegedly show that Israel was interested in enabling Hamas activity in its beginning, intending to weaken the Palestine Liberation Organization and ending the first Intifada.  Hamas was established in 1987, and has its origins in Egypt’s Muslim Brotherhood that the United States General Patraeous, the CIA  Creatures of Jesuit Papacy and the Knights of Malta helped to …Israel and Egypt then imposed an economic blockade on Gaza.  Even US Congressman Ron Paul spoke about how  Israel created Hamas to destabilize Arafat who was very powerful at the time.

Thanks to the Mossad, Israel’s “Institute for Intelligence and Special Tasks”, the Hamas was allowed to reinforce its presence in the occupied territories. Meanwhile, Arafat’s Fatah Movement for National Liberation as well as the Palestinian Left were subjected to the most brutal form of repression and intimidation.  Let us not forget that it was Israel, which in fact created Hamas. According to Zeev Sternell, historian at the Hebrew University of Jerusalem, “Israel thought that it was a smart ploy to push the Islamists against the Palestinian Liberation Organisation (PLO)”. The Hamas had built its strength through its various acts of sabotage of the peace process, in a way which was compatible with the interests of the Israeli government. In turn, the latter sought in a number of ways, to prevent the application of the Oslo accords. In other words, Hamas was fulfilling the functions for which it was originally created: to prevent the creation of a Palestinian State. And in this regard, Hamas and Ariel Sharon, see eye to eye; they are exactly on the same wave length. 

A majority of the worlds problems, from poverty to world wars, are the cause of International “satanic secret society,” Banking cartel.  Many or the Worlds religions and their representatives are secretly being controlled by Secret Satanic Society’s such as the most famous Freemasons who have been influencing and trying to control secular society with a Satanic agenda that Albert Pike (December 29, 1809 – April 2, 1891) wrote about in a book he published called Morals and Dogma of the Ancient and Accepted Scottish Rite of Freemasonry in 1871. Albert Pike was an American attorney, Confederate officer, writer, and Freemason. Albert Pike wrote a letter to Giuseppe Mazzini, dated August 15, 1871.  This letter graphically outlined plans for three world wars that were seen as necessary to bring about the One World Order, and we can marvel at how accurately it has predicted events that have already taken place.

Pike’s Letter to Mazzini: It is a commonly believed fallacy that for a short time, the Pike letter to Mazzini was on display in the British Museum Library in London, and it was copied by William Guy Carr, former Intelligence Officer in the Royal Canadian Navy. The British Library has confirmed in writing to me that such a document has never been in their possession. Furthermore, in Carr’s book, Satan, Prince of this World, Carr includes the following footnote:

“The Keeper of Manuscripts recently informed the author that this letter is NOT catalogued in the British Museum Library. It seems strange that a man of Cardinal Rodriguez’s knowledge should have said that it WAS in 1925”.

It appears that Carr learned about this letter from Cardinal Caro y Rodriguez of Santiago, Chile, who wrote The Mystery of Freemasonry Unveiled.

To date, no conclusive proof exists to show that this letter was ever written. Nevertheless, the letter is widely quoted and the topic of much discussion.

Following are apparently extracts of the letter, showing how Three World Wars have been planned for many generations.

“The First World War must be brought about in order to permit the Illuminati to overthrow the power of the Czars in Russia and of making that country a fortress of atheistic Communism. The divergences caused by the “agentur” (agents) of the Illuminati between the British and Germanic Empires will be used to foment this war. At the end of the war, Communism will be built and used in order to destroy the other governments and in order to weaken the religions.” 2

Students of history will recognize that the political alliances of England on one side and Germany on the other, forged between 1871 and 1898 by Otto von Bismarck, co-conspirator of Albert Pike, were instrumental in bringing about the First World War.

“The Second World War must be fomented by taking advantage of the differences between the Fascists and the political Zionists. This war must be brought about so that Nazism is destroyed and that the political Zionism be strong enough to institute a sovereign state of Israel in Palestine. After this Second World War, Communism was made strong enough to begin taking over weaker governments. Since the terrorist attacks of Sept 11, 2001, world events, and in particular in the Middle East, show a growing unrest and instability between Modern Zionism and the Arabic World. The White Horse Prophecy is a statement purported to have been made in 1843 by Joseph Smith, Jr., founder of the Latter Day Saint movement, regarding the future of the Latter Day Saints (Mormons) and the United States of America. The Latter Day Saints, according to the prophecy, would “go to the Rocky Mountains and … be a great and mighty people”, identified figuratively with the White Horse described in the Revelation of John. The prophecy further predicts that the United States Constitution will one day “hang like a thread” and will be saved “by the efforts of the White Horse”.[1]

Wait, isn’t Dr. Ron Paul out of the presidential race? Isn’t it all tied up nicely in a bow with the Romney/Ryan ticket?

No.

Why would the GOP be scared of Ron Paul but end up nominating him?

I’ll explain.

Romney and the GOP have demonstrated both poor judgment and poor sportsmanship that might cost them by damaging Romney’s electability among the Ron Paul supporters thus leading to a splitting of votes, which in turn, could cost the GOP the entire election.

Dr. Ron Paul is still in the race for president and is a strong contender for the 2012 GOP nomination.

To be on the GOP ballot Aug. 27, 2012 in Tampa and get a 15-minute speaking slot, a candidate must have won the plurality (majority) of delegates in at least 5 states.

Well, Ron Paul did win the plurality of delegates in 5 states, enough to be eligible for the nomination and a 15-minute speaking slot at the GOP convention. The states he won are Louisiana, Iowa, Minnesota, Maine and Nevada. Then Ron Paul went on to win the plurality in Massachusetts, Romney’s home state and half the delegates in Oregon. Dr. Ron Paul also has around 500 delegateswho support him. The exact number of delegates that Romney and Paul have is still a mystery but should be clarified at the convention.

So… Ron Paul won his 5-plus states, he’s on the ballot and writing his speech, right? Not exactly.

What happened next is what may cost the Republicans and Romney the entire election. Instead of accepting that Ron Paul, the GOP underdog, had won enough delegates in enough states to be allowed his rightful place on the ballot and his 15-minute speaking slot, the GOP and Romney’s people decided to try and take these legitimate wins away from Ron Paul and his supporters. Ron Paul supporters fought hard, played fair and won. Romney supporters didn’t play fair and still lost those 5-plus states. These Ron Paul pluralities were won in spite of shenanigans and tricks tried by Romney supporters and the GOP to prevent or undermine Ron Paul wins. The Ron Paul supporters were well prepared and won the needed amount of states anyway.

So, how did Romney and his supporters handle their losses of five measly states to Ron Paul? Honorably? Graciously? With dignity? Maturely?

No. Quite the opposite.

Romney’s people ran to Big Daddy GOP to rescue them from their defeats by trying to disqualify the valid Ron Paul delegates and to take away Ron Paul’s right to speak and be on the ballot by reducing his states won to fewer than the five needed.

So far, Romney and the GOP have contested the Ron Paul wins in Louisiana, Maine, Massachusettsand Oregon. They threw out the Massachusetts Ron Paul delegates after the GOP tried to force the delegates to sign a long legal document that required them to vote for Romney. This was not something that had ever been done before. The GOP allowed Romney, big lawyers and big bullying to invalidate Ron Paul’s solid win in Massachusetts.

As in the other states, the Ron Paul delegates in Maine played by the rules and won. Even Governor LePage of Maine, a Republican, is appalled with GOP efforts to throw out the duly-elected Ron Paul delegates.

The entire plot to reduce Ron Paul wins to under five states to take his name off the ballot and take away his 15-minute speaking slot is well under way. If Romney/Ryan are so great, why can’t they handle a little competition without whining and crying like sore losers?

Are they afraid of losing the 2012 nomination to Ron Paul if he speaks and is on the ballot?

Why not play fair and let the best man win?

Who cheats? The lazy, bratty, insecure and less-qualified people who can’t win honestly and fairly. Is that really presidential material?

I wonder if Romney and the GOP are worried that if Ron Paul speaks and gets the nomination, that Ron Paul will fire all of them. Is the GOP that terrified of change, even if it is for the betterment of the country?

Here is why I think that Ron Paul might still walk away with the nomination.

4. Lots of regular “little people”, like me, want Ron Paul to be the 2012 GOP presidential candidate. We, the little people, do not like being trod upon and when we join together, we are stronger than big money. Remember, “We the People”? Ron Paul’s donors areregular working people, many military men and women, not banks.

Ron Paul is the David to Romney’s Goliath Machine but the spirit, character and faith of we “little people” count more in America than just money. Romney has raised$152 million to Paul’s $39 million. Ron Paul has more passionate support from his supporters due to Ron Paul’s character and dedication to serving America and Americans based on our founding principles embodied in our Constitution. Money, media manipulation and bullying can’t buy this type of support. But, Romney and his lawyers couldn’t let Ron Paul keep 5-plus wins?

2. Ron Paul wants to serve his country, and has served in the military, and would end the wars. Ron Paul’s supporters and Americans want someone they can trust and believe in. Dr. Paul is that man.

1. Mitt Romney is much less qualified to be president than Ron Paul, who understands the Constitution and Bill of Rights. Watch this link where Romney is asked a basic Constitutional question and replies, “I’ll have to ask my lawyers”. Ron Paul tells Mitt Romney, “Read the Constitution.” Ron Paul is the only Republican Presidential candidate who is qualified, experienced and credible.

We, little people, similar to Ron Paul and his supporters, work hard, play fair and expect the same from others.

If Romney and his supporters had been gracious losers about Ron Paul’s 5-plus state wins and Ron Paul’s 15-minute speech, Romney had a chance to garner both the GOP 2012 nomination plus the support and votes of Ron Paul and his supporters. (I say “a chance” because many Ron Paul supporters do not see Romney as a fiscal conservative.)

Not anymore. It is hard to vote for someone who has acted so dishonorably, even for the sake of party loyalty.

The misguided Machiavellian advice Romney got and took to “win at all costs,” may cost him dearly in November due to the bad will generated with Ron Paul supporters. Too many people have lost too much respect for Romney to vote for him. To the contrary, even though Ron Paul’s campaign was repeatedly slighted and undermined by Romney and his supporters, Ron Paul stayed gracious and denounced mistreatment of Romney. He has told his delegates to “Make yourself heard but be respectful.”

By playing dirty and being greedy, Romney supporters may have lost Romney both the nomination and the election.

Romney and the national GOP are showing their ugly colors by attempting to take away Ron Paul state delegate wins that were earned fair and square.

In speaking with some Ron Paul supporters, here is their current plan. If Ron Paul doesn’t win the 2012 GOP nomination, many Ron Paul supporters will elect to vote for Gary Johnson, the Libertarian presidential candidate. Why wouldn’t Ron Paul supporters write in Ron Paul on ballots around the country? For those write-in votes to count, a write-in candidate must be listed in every state and some Ron Paul supporters do not know if their Ron Paul votes would count.

And, due to the shabby treatment Ron Paul and his supporters have received from the GOP and Romney’s people, many Ron Paul supporters may refuse to vote for Romney in 2012, even if it means Obama being re-elected for four more years. One supporter told me, “I want the GOP to see how many votes they lost by playing dirty. They will only be able to count all the lost votes if I vote for Gary Johnson.”

He is like Ron Paul without the Roe v Wade concern. Gary Johnson has had virtually no media coverage so you may not have heard of him.

As I heard in grammar school and it remains true: “Cheat, Cheat Never Beat”.

Ron Paul is the only qualified person for the 2012 Republican Nomination who can unify conservative Republicans, Independents, Libertarians and stabilize the economy. If the GOP is smart and humble enough to do a mea culpa and nominate Ron Paul, Republicans stand a better chance of winning in November. If Ron Paul doesn’t get the nomination, many Ron Paul supporters will likely be voting for Gary Johnson. All Romney and the GOP had to do was play fair and win honorably, but I guess that was too much to ask.

But here is the GOP’s conflict: If the GOP nominates Ron Paul and he wins, many of the GOP cronies will be shown the door. There will be a big house cleaning of corrupt politicians, like turning on the lights and the cockroaches scatter. On the other hand, if they don’t nominate Ron Paul, they risk losing all the Ron Paul supporters, which would likely split the vote and lose the 2012 election entirely.

It is hard to make those type of decisions when choosing what is best for the country doesn’t figure in at all, but choosing what is best for their own selfish principles of self-preservation, greed and thirst for power rule the day.

Romney and the GOP need to leave Ron Paul’s five-plus states alone, place him on the ballot and allow Ron Paul to speak.

My name is Douglas A. Mutschler CW2, USA (ret) and during my service tour in Alaska I was informed of a pyramid under the land in Alaska. There is other information concerning this that came to my attention after it was reported to us in 1992. I have tried to pass this info to others but have not heard any response to my information. I assure you I am telling you the truth about this and I think that this is being kept quiet by our government as the news was buried the very next day after it was brought to my attention. The fifth angel sounds his trumpet and a star falls from heaven. This star has the key to the bottomless pit. Stars sometimes have a double reference to angelic beings in the scriptures. This star is called “him” so it is an angel. The angel was given the key to open up the place under the earth where fallen angels are bound (2 Pe: 2 1-10). Some think this angel is Satan and others think it is another angel. I think this angel still has the key when he locks Satan in the bottomless pit for a thousand years after the tribulation is over (Rev 20:1-2). Therefore, the angel is from God and is not Satan.

It is too soon for Satan to be cast out of heaven permanently since the Beast he incarnates when he is cast out of heaven comes out of the pit months after the time of this fifth trumpet. The Beast comes out of the pit just prior to the seventh trumpet just before he kills the two prophets who have completed their 1,260-day testimony (Rev 11:7).

If this was actually the time when Satan and his angels were cast out of heaven by Michael and his angels there should be a description of more than one star falling from heaven. This angel appears to be on an assignment and does not appear to be an angel being banished from heaven.

Although it is clear that this star is a personality, I do not think we can say that the comet had nothing to do with opening the pit in a physical sense. Physical and spiritual events can be interrelated. Mankind being limited to four dimensions cannot understand the mystery of how the physical and the spiritual are interrelated (Mat 16:19, Mat 18:18). These physical judgments on earth are the end result of spiritual prayers. We read in the last chapter that a censor containing the prayers of the saints was filled with fire by an angel. This angel cast the censor upon the earth and then these physical judgments came.

When the comet hits the earth it will penetrate the crust of the earth and open the bottomless pit. We see this physically happens on earth by the description of the great smoke that comes out of the pit that darkens the skies. The opening of the bottomless pit provides a way out for devils or demons that have been bound in the pit (by gravitational forces perhaps?).

 

3 And there came out of the smoke locusts upon the earth: and unto them was given power, as the scorpions of the earth have power.

The bottomless pit is opened and tormentors come on the earth. Their satanic role probably is to make life on earth so unbearable that a counterfeit savior will be accepted. Out of the smoke of the pit comes a strange type of locust upon the earth. These locusts have power like scorpions. What these beings are is not certain and I have no natural explanation. The stinging locusts last on the earth for five months. We can be sure they are demonic beings because they come out of the bottomless pit and we are told that they will have a demonic king ruling over them.

 

4 And it was commanded them that they should not hurt the grass of the earth, neither any green thing, neither any tree; but only those men which have not the seal of God in their foreheads.

5 And to them it was given that they should not kill them, but that they should be tormented five months: and their torment was as the torment of a scorpion, when he striketh a man.

These locusts were commanded by God not to hurt anything green so these are not  normal locusts or any natural mutation. They sting only those who do not have the seal of God in their foreheads. The 144,000 most likely are not the only ones sealed. Those who believe and follow Jesus may also be sealed since they are also servants of God. Scripture does not tell us that believers will be sealed but I see no reason for God to let believers to be tormented by demonic beings. The fact that they are commanded and cannot harm those sealed by God say that these beings are intelligent and have no choice but to obey spiritual authority given to the angels of God.

These locusts will not be given power to kill anyone but they will have the power to torment unsealed people for five months. Some make this judgment a result of nuclear warfare. They claim that this judgment is talking about radioactive poisoning but many who suffer this do die. It is obvious from the description of these locusts that this is not what is being described. Some say that they are spiritual demons that torment man mentally but passages that follow indicate that these beings have physical form. The scorpion sting is said to be the most painful of all stings. Those on earth who are not sealed have this type of pain to look forward to for five long months.

 

6 And in those days shall men seek death, and shall not find it; and shall desire to die, and death shall flee from them.

This is going to be so bad that people will seek death but they will not find it. Why man cannot find death is unknown but God gives and takes life. Perhaps, because the pit of hell is opened, no one can die and go there until after the five months is up. It may be 5 months of the real Night of Living Dead for people on earth.

God, for His own reasons is not allowing any unsealed person to escape this torment. I think this is the ultimate tough love program. I believe God is allowing demons to give men of flesh a five month taste of the Lake of Fire. It is given in love so they will choose not to go there for eternity when they later are required to make a choice between taking the mark of the Beast and going to this place of torments for eternity or choosing God’s Son and losing their physical lives so their soul will be saved from it. All mankind will still have another chance to come to Jesus and not choose to go to the place of eternal torments.

 

7 And the shapes of the locusts were like unto horses prepared unto battle; and on their heads were as it were crowns like gold, and their faces were as the faces of men.

8 And they had hair as the hair of women, and their teeth were as the teeth of lions.

This passage gives the actual physical description of these creatures. They are not like anything we know. They are not helicopters or jets shooting missiles filled with chemical or biological weapons as some have speculated. These creatures come out of the smoke of the pit and the angel of the bottomless pit is ruling over them. This is not Satan because Satan at this point has not yet been cast out of heaven.

The king over these beings is named Abaddon. The name Abaddon actually means the destroyer. These locusts are led by the destroying angel who is probably Satan’s commander of the dimension called the underworld. These locusts are actual physical beings that ascend out of the same pit the angel in verse two opened with his key. The five months of this torment will be a literal hell on earth.

 

12 One woe is past; and, behold, there come two woes more hereafter.

After the five months of torment by these hellish creatures, one woe is past but there are still two more left to come when the sixth and seventh angels sound their trumpets.

 

13 And the sixth angel sounded, and I heard a voice from the four horns of the golden altar which is before God,

14 Saying to the sixth angel which had the trumpet, Loose the four angels which are bound in the great river Euphrates.

15 And the four angels were loosed, which were prepared for an hour, and a day, and a month, and a year, for to slay the third part of men.

16 And the number of the army of the horsemen were two hundred thousand thousand: and I heard the number of them.

After the sixth trumpet blast, four angels are loosed that are bound in the Euphrates River. This prophecy gives the number of the horsemen (cavalry men) that will now be allowed to pass to the east of the Euphrates to kill one third of men. The number John said he heard was two hundred million. There never was a time when such an army was possible before this generation.

Today such an army could come from the East; there are now more than three billion people east of the Euphrates River. The CIA fact book of the year 2004 said that China alone has more than two hundred million military fit men.

Abortion and infanticide of female children in China and India in this generation will create a disposable surplus of over one hundred million adult men who will have no chance for marriage and a normal family life. To a lesser extent for much the same reason there will also be surpluses of men in other countries of the Far East. It is as if these men were born for a special time to fulfill this prophecy. That is exactly what the wording of this passage implies. There will be almost enough adult men to make up this army from just the surplus males of military age that will exist in China and other Far-East nations by 2035 AD.

Some commentators think that this is an army of demons. In all cases, demons seek to possess bodies so a reasonable compromise is that two hundred million demons will come out of the pit and possess, and control this two hundred million-man army. This army will cross the Euphrates and come into the Middle East and the West. This army will kill one third of all men that survived the previous disasters on the earth. By the time the prior judgments and this second woe is completed over half of the world’s population will be dead.

Some commentators are hung up on the word horsemen thinking that all the men would have to be riding horses. Since these believe it is not possible to assemble two hundred million horses, they dredge up demons that look like men on horses. More likely, the wording simply means the leaders are mounted on horses or the men travel on something mobile (i.e. military vehicles).

 17 And thus I saw the horses in the vision, and them that sat on them, having breastplates of fire, and of jacinth, and brimstone: and the heads of the horses were as the heads of lions; and out of their mouths issued fire and smoke and brimstone.

18 By these three was the third part of men killed, by the fire, and by the smoke, and by the brimstone, which issued out of their mouths.

19 For their power is in their mouth, and in their tails: for their tails were like unto serpents, and had heads, and with them they do hurt.

The description in this passage is probably only supernatural in the sense that angels were loosed for a specific time to allow this to happen and that demons are now free to deceive and inhabit those who dwell on the earth. The entire wording of this judgment makes it fairly clear to me that this is describing a great all out world war between the West and the East.

The fire that comes out of the mouths of the horsemen is every modern weapon of warfare like tanks, and artillery. The tails like serpents that had heads that did harm sound exactly like modern rockets and bombs with warheads. This will be an all out world war using nuclear, chemical and biological warfare and billions will die.

 

20 And the rest of the men which were not killed by these plagues yet repented not of the works of their hands, that they should not worship devils, and idols of gold, and silver, and brass, and stone, and of wood: which neither can see, nor hear, nor walk:

21 Neither repented they of their murders, nor of their sorceries, nor of their fornication, nor of their thefts.

Even after mankind uses all the weapons of warfare they made with their own hands against one another they still will not repent of the works of their hands. Instead of trusting in God to save them, they trust and worship the created instead of the Creator. Most of the East will still pray to statues that represent Buddha or countless other demon gods. Instead of obeying God’s servants on earth, they will follow doctrines given by demons. Instead of coming to God to be changed and to be given a new heart they will continue in their insane practices. Instead of worshiping the God of heaven, they will now worship the image of the Beast possessed by Satan.

Because the world did not want to believe the truth, they will now be given a lie and they will accept Satan’s counterfeit savior and his counterfeit kingdom (2Th 2:11). This deception happens because these still love to murder, use drugs, commit adultery and steal. They love evil and that is why they will not receive the truth and be saved.

Thanks Daniel & Elbee, One if the biggest problems that I can see- is that what was once called a Neurobehavior Disorder-years earlier has somehow now been coined a neuro ‘psychiatric’ disorder. An example of a reason-why it is so important to know when a person has a Post Strep movement disorder – is that; for example what happened to a boy on my son’s hockey team -if a doc ever wrongly prescribes a stimulant i.e. ritalin for ADHD- that person can have life long devastating effects…seizures, limbs twisted in contortions. Left untreated- Strep can cause more than rheumatic fever. My son a couple of years ago -got a bit run down, and missed sleep playing soccer- yet, with a negative for strep throat swab, he went into kidney failure- later a blood titre 300 times too high -showed it actually was Strep related. Watch someone suffer in horrific pain, on a palliative unit- and the very thought of this being called a conversion/ fake disorder– is disgusting.

As mentioned previously, the New York State Department of Health was given a medical record copy to prove what I am saying of the Strep titre / Neurologist who saw my son re: PANDAS 12 years ago/ and the kidney failure…..one would assumethe ‘right’ people have already seen it. The kids ought to be given an antibiotic at any rate….in case it helps their symptoms subside some. Personally, I’ve about given up trying to make it easy for Specialists there who are supposed to already know all this.

ABSTRACT

Clinicians have been faced with much publicity and contradictory scientific evidence regarding a recently described condition termed pediatric autoimmune neuropsychiatric disorders associated with streptococcal infection (PANDAS). It has been proposed that children with PANDAS experience tics, obsessive-compulsive behavior, and perhaps other neuropsychiatric symptoms as an autoimmune response to streptococcal infection. We review current scientific information and conclude that PANDAS remains a yet-unproven hypothesis. Until more definitive scientific proof is forthcoming, there seems to be insufficient evidence to support 1) routine microbiologic or serologic testing for group A streptococcus in children who present with neuropsychiatric symptoms or 2) the clinical use of antibiotic or immune-modifying therapies in such patients. The optimum diagnostic and therapeutic approach awaits the results of additional research studies.

With a description of 50 patients, Swedo et al1 proposed a hypothesis that childhood obsessive-compulsive disorder (OCD) and/or tics may arise as a result of a poststreptococcal autoimmune process; the authors suggested the acronym PANDAS (pediatric autoimmune neuropsychiatric disorders associated with streptococcal infections). It additionally was suggested that, by a process of molecular mimicry, somatic epitopes of the group A β-hemolytic streptococcus (Streptococcus pyogenes) (GABHS) evoke antibodies that are capable of cross-reacting with specific areas of the human brain (eg, the basal ganglia) to produce neuropsychiatric and behavioral symptoms. The authors hypothesized that this condition might be a forme fruste of the pathogenetic process thought to underlie Sydenham’s chorea and therefore also may be related to rheumatic fever.2,3 The observation that Sydenham’s chorea includes tics4 and sometimes obsessive-compulsive symptoms5contributed to the proposal that a poststreptococcal autoimmune mechanism(s) might lead to a more diverse neurobehavioral symptom spectrum. Subsequently, the clinical spectrum associated with this constellation of neuropsychiatric signs and symptoms (PANDAS) was proposed to include a number of additional manifestations including attention-deficit/hyperactivity disorder,6 myoclonus,7 dystonia,8 paroxysmal dyskinesias,9 acute disseminated encephalomyelitis,10 and anorexia nervosa.11

This PANDAS hypothesis has stimulated clinical and basic research and has resulted in considerable clinical and scientific controversy12–14; the controversy has influenced clinical care. Because of the implications for the primary care physician, the neurologist, the psychiatrist, the infectious disease specialist, the cardiologist (if it is to be considered part of the rheumatic fever complex, as is Sydenham’s chorea), and even the basic scientist, it is appropriate to objectively examine available information regarding the hypothesis and to provide recommendations for a current, optimal clinical approach to patients suspected of having PANDAS.

THE DIAGNOSTIC CRITERIA: IS PANDAS AN ADEQUATELY DEFINED CLINICAL DIAGNOSIS?

Five criteria were proposed to define the PANDAS hypothesis1 (see Table 1).

Criterion 1

As mentioned, several authors have suggested that tics and OCD may not be the only clinical features of PANDAS, and thus the true boundaries of the PANDAS clinical spectrum remain to be scientifically validated or standardized. Even considering only tics and OCD, the level and characteristics of symptom severity required for diagnosis have not been defined yet by controlled studies.

Criterion 2

The proposed age at onset, ranging between 3 years and the beginning of puberty, may be too arbitrary and may simply be the result of referral patterns for the 50 initially described patients. For example, the validity of the proposed age-at-onset criterion is raised by the report of a 25-year-old with sudden-onset OCD after pharyngitis.15 Furthermore, because the vast majority of typical Tourette’s syndrome (TS) manifestations (ie, tics) begin during the prepubertal period (75% having onset before 11 years old16), the age-at-onset criterion is not sufficiently specific in distinguishing PANDAS from the more established diagnostic criteria for TS.17 Recognizing a documented higher likelihood of exposure of schoolchildren to GABHS, no pathogenetic or immunologic basis has been confirmed to explain why the onset might be expected to include only young children.

Criterion 3

A unique specificity of a clinical course consisting of abrupt onset or dramatic exacerbations has not been documented adequately. With careful observation, tics are either present or not; thus, the onset (or parental awareness) of tic disorders is not gradual. Often with a sudden recognition of tics by parents, in retrospect a prior history of milder tics can be elicited, making the precise timing of clinical onset confusing. Experienced neurologists and psychiatrists are aware that tic disorders and OCD characteristically wax and wane spontaneously, with exacerbations followed by remissions. The degree of clinical change required for recognizing exacerbations to distinguish a “PANDAS-like process” from the typical course of more established tic and OCDs has not been differentiated or quantitated adequately. For example, 1 recent study of 80 consecutively examined (unselected) patients in a tic-disorders clinic found that 53% reported sudden, explosive onset or worsening of their tics.18 These patients were not considered to have met criteria for a diagnosis of PANDAS. Preliminary evaluation of our own ongoing, case-control epidemiologic study of PANDAS indicated that 36% of the carefully selected control subjects (who had no recognized link between symptoms and GABHS infection) also reported an abrupt onset or dramatic exacerbations (R. Kurlan, MD, unpublished data, 2003). Thus, clinical course does not seem particularly useful in distinguishing patients suspected of PANDAS from children with more typical cases of TS or OCD.

Criterion 4

Onset or exacerbations of neuropsychiatric symptoms temporally related to GABHS infection has been proposed as the sine qua non of the diagnosis of PANDAS. Yet, despite limited studies to establish this, it has not been documented adequately in prospective studies and remains 1 of the most controversial aspects of the hypothesis. GABHS infections are very common, and the prevalence of this organism in the upper respiratory tract of asymptomatic school children (“carriers”) often is relatively high.19 This epidemiologic phenomenon of high prevalence of GABHS has resulted in a number of other disease entities (among the best examples are Henoch-Schönlein purpura and Kawasaki disease), which also have been proposed to be associated with group A streptococcal infections. However, when adequately controlled studies were conducted, streptococci were eliminated as the causal factor.20 The confusion about the role of GABHS may be promoted further because, clinically, it is appreciated that tic disorders and OCD worsen during times of stress or illness of any kind. The worsening of symptoms might be expected with streptococcal upper respiratory tract infection and could even occur in the relatively large percentage of children who are asymptomatic streptococcal carriers if simultaneous stressful situations occur (from any other cause).

Additionally, review of published studies indicates that the temporal boundaries of a proposed “temporal” relationship with streptococcal infections have never been established. In Sydenham’s chorea, often cited as the model for PANDAS, the choreoathetoid movements typically appear 3 to 5 months after GABHS infection. Swedo et al1 implied that an infection up to 9 months before symptom onset may be acceptable for the diagnosis of PANDAS but also have indicated that the presence of GABHS in the upper respiratory tract (not differentiating infection from the carrier state) may not be detectable until weeks after onset or exacerbation. Such a lack of precision does not assist in establishing a distinct syndrome.

Thus, the absence of regular, continuous, prospective throat cultures along with streptococcal antibody determinations has made it impossible to establish conclusively a temporal relationship between streptococcal infection and onset or exacerbation of neuropsychiatric symptoms. Sufficiently long, longitudinal, prospective studies conclusively demonstrating streptococcal infection and correlating this with clinical findings are required.

Criterion 5

The final diagnostic criterion, that there must be abnormalities on neurologic examination, presents a difficult and practical clinical problem. For example, the 1998 report that “choreiform” movements are often present in patients in whom a diagnosis of PANDAS is considered indicates that the cohort on which the proposed diagnostic criteria were based may well have included subjects with mild chorea that might also be compatible with more conventional Sydenham’s chorea.1

CLINICAL MANAGEMENT OF PATIENTS HAVING SOME OR ALL OF THESE NEUROPSYCHIATRIC MANIFESTATIONS

Laboratory Testing

An initial concern of pediatricians and primary care physicians about clinical management of patients with this symptom complex is which, if any, diagnostic laboratory studies should be done. Given the current status of as-yet incompletely defined support for the existence of a specific entity called PANDAS, the physician may be cautious in deciding whether to obtain throat cultures or determine antistreptococcal antibodies in all children presenting with neurobehavioral symptoms such as tics, obsessions, or compulsions. Because of potential difficulties in interpretation of single tests, even with “positive” culture results, the high prevalence of so many GABHS carriers in normal populations potentially confuses the issue.

Patients with signs and symptoms compatible with streptococcal upper respiratory tract infections should be studied for the presence of GABHS just as with any other patient. In the absence of symptoms of streptococcal infection, the relevance of a positive culture is difficult to interpret. The many uncertainties encountered in interpreting streptococcal antibody titers in otherwise normal children are recognized.21 Therefore, to use streptococcal antibody tests or throat cultures in an asymptomatic child either as a screening test or to confirm a diagnosis of PANDAS is problematic.

Two research laboratory tests, measurement of circulating antineuronal antibodies and an assay for the presence of an alleged rheumatic fever-associated T lymphocyte alloantigen called D8/17,22 have not been able to correlate reliably and consistently with suspected PANDAS cases.14

Although radiologically suspected enlargement of caudate nuclei has been described in some suspected patients, routine magnetic resonance imaging cannot be recommended currently, because the validity has not been substantiated.23

Therapy

What treatment should be considered for patients who present with the manifestations associated with the PANDAS constellation of symptoms?

Neuropsychiatric Drugs

Children with disabling tics or OCD should receive appropriate treatment for their symptoms, including medications (eg, tic suppressants such as α-agonists, classical or atypical antipsychotics, or antiobsessional drugs such as selective serotonin-reuptake inhibitors) or nonpharmacologic approaches (eg, cognitive behavioral therapy for OCD).24

Antibiotics to Prevent Strep Infections

The only published clinical trial of prophylactic penicillin in children with this syndrome revealed no conclusive evidence that the antibiotic reduced clinical exacerbations.25 Admittedly, the duration of that study was too short to allow definite conclusions. A later report suggesting improvement of new-onset or acute exacerbations of symptoms in such children with antibiotics provides inadequate support for such an approach, because treatment was not placebo-controlled and was unblinded.26 It is well known in treatment studies of TS that there is a substantial placebo effect; the natural course of TS and OCD is such that exacerbations are followed by remissions. This latter phenomenon of “reversion to the mean” implies that virtually any intervention at the time of peak symptoms may seem successful. Only a double-blind, placebo-controlled study can identify a true therapeutic effect.

Another reason to feel comfortable with avoiding antibiotic treatment for these patients is that, to date, no cases have been reported to develop any rheumatic carditis as occurs in patients with Sydenham’s chorea. This is a very important point to remember, because some have attempted to relate this syndrome to rheumatic fever. It is recognized that as many as one third of patients with Sydenham’s chorea ultimately will have evidence of rheumatic valvular heart disease.27 This issue requires additional investigation, because the lack of heart disease strongly argues against a relationship between PANDAS and Sydenham’s chorea or other forms of rheumatic fever.

IS THERE A CASE FOR IMMUNOMODULATORS?

Because the postulated mechanism for a PANDAS syndrome implies an immunologic dysfunction, therapeutic trials have attempted to modify the effects of an immune response. A published study involving 29 patients fulfilling suggested criteria for PANDAS reported sustained benefits both after plasma exchange and in patients after intravenous immunoglobulin.28However, Singer,29 in an accompanying commentary, pointed out numerous methodologic concerns about the trial including highly selective recruitment, small sample size, lack of severity matching within treatment groups, limited comparisons with controls, absence of sham apheresis, concomitant use of psychotropic medications, possible treatment-order effects, and adverse effects that occurred in approximately two thirds of subjects receiving active treatment. A subsequent note of caution was issued in 2000 by the National Institute of Mental Health recommending that, at this time, plasma exchange and intravenous immunoglobulin be reserved only for subjects participating in approved research protocols because there are distinct risks involved.30 To date, there are no additional published data to encourage use of these immune-modifying techniques.

CONCLUSIONS

The current state of knowledge dictates that the concept of a constellation of neuropsychiatric signs and symptoms that are associated with GABHS infections (the PANDAS hypothesis) should be considered only as a yet-unproved hypothesis. History reminds us that the same autoimmune hypothesis for childhood autism, including poststreptococcal elements,31 has been pursued for decades32 without conclusive proof. The PANDAS hypothesis is the focus of clinical and laboratory research. There currently is insufficient evidence to support routine laboratory testing or antibiotic/immune-modifying treatment for suspected cases. Although there is great need for a more effective diagnostic and therapeutic approach for these children, the clinician is obliged to await additional data from well-controlled prospective studies to determine whether PANDAS is a documentable entity or will remain an unproven hypothesis.
